Aging Life Care Manager
Aging Life Care Manager. Bachelor’s degree in gerontology, social work, nursing, or relevant human services required, Master’s degree preferred. Care/case management experience and certification preferred. Excellent organizational and communication skills. Ability to work independently and as part of a team. Local daily travel required predominantly in Pinellas county. Excellent opportunity to support a small caseload of older/disabled adults and their families navigate the health & human services systems through consultation/assessment to facilitate informed decisions; provide oversight of care; and make proactive recommendations to manage changes occurring with aging.
